A player messaged me asking why a level 28 could be wearing a level 30 item.

obviously this is because he went inactive and lost levels from being attacked.  The items don't go back into his backpack or anything..........but even if they did, what if their backpack is full? where would the items go

If they did, they would go into backpack anyways, doesn't matter, if it's full or not. In former case, player just can't pick up any items until he has gotten rid of the items.
